Update the info array to change each card's title, icon, and lines, including multiple address or email lines per card. Fields use daisyUI input, select, textarea, and btn btn-primary classes and follow your theme. The columns stack on mobile and split on large screens. Hook the form to your own handler since it is static markup. It is Tailwind CSS and React, ready to copy and paste into a Next.js project.
