The form fields use daisyUI input, textarea, and btn btn-primary classes, so they pick up your theme automatically and you can add fields by copying a form-control label. The two columns stack on mobile and split on large screens. Wire the form to your handler or backend, since it ships as static markup. Built with Tailwind CSS and React for easy copy and paste into Next.js.
